The problem type will help the IT department determine the best technician to solve your request. If a problem type does not meet any of the selectable choices for your request, please choose 'Unknown". please feel free to let us know so we can make the selections better to help you!


 

Computer Name and/or Room Number and/or General Location

This is not required but could be extremely useful in solving your issue more efficiently. the computer name can be found by looking at the desktop of your computer where all of the icons are. Locate the "Computer" or "My Computer" icon. the computername will be listed directly under that icon. It will begin with the location you are in.

If the issue is non-computer related, a room number or general location will be most useful to get a technician to you.
